Privacy Policy
How we collect, use, and protect your personal health information — in full compliance with Ontario's PHIPA legislation.
Effective Date: September 2026 | This policy applies to Addiction Clinic Toronto (Road to Recovery), located at 55 Dundas St E, Toronto, ON M5B 1C6.
At Addiction Clinic Toronto (Road to Recovery), we are committed to protecting the privacy and confidentiality of our patients' personal health information. We operate in full compliance with Ontario's Personal Health Information Protection Act, 2004 (PHIPA). This Privacy Policy explains what information we collect, how we use it, and your rights as a patient.
1. Collection of Information
We collect personal health information (PHI) only as necessary to provide you with safe, effective medical care and treatment. This may include:
- Contact information (name, address, phone number)
- Health history and Opioid Agonist Treatment (OAT) records
- OHIP number, for billing purposes
- Clinical visit notes and laboratory results
- Medication records and prescription history
2. Use and Disclosure of Information
Your health information is used within your Circle of Care to ensure you receive the best possible treatment. We may share your information with:
- Healthcare providers directly involved in your treatment (e.g., your family physician, or the pharmacy you choose to fill your prescription)
- The Ministry of Health, for OHIP billing purposes
- Regulatory bodies such as the CPSO, when required for safety or legal compliance
We will never sell or share your personal health information with third-party marketers or unauthorized individuals without your express written consent.
3. Your Rights
Under PHIPA, you have the following rights regarding your personal health information:
Access
Request a copy of your personal health record at any time.
Correction
Ask us to correct any information you believe is inaccurate or incomplete.
Consent
Withdraw your consent for certain uses of your information at any time.
4. Security Safeguards
We take the security of your health information seriously. We protect your data through a combination of:
- Secure Electronic Medical Records (EMR) systems with restricted access
- Staff confidentiality agreements and privacy training
- Physical security measures at our clinic premises
- Administrative protocols limiting who can view patient files
Only authorized clinical staff who need access to your information to provide care will ever view your records.
5. Our Website
This section is about addictionclinictoronto.ca, the website itself. It is separate from the clinical information described above: the website has no access to our Electronic Medical Record, and nothing described in this section is added to your chart.
Website measurement. So that we can tell which pages people actually use, and whether a visit leads to a phone call, each page view, each contact-form submission, and each tap on our phone number, email address or map link records four things:
- The address of the page on this site, with anything after a “?” removed
- The address of the page that linked you here, if any, also with anything after a “?” removed
- A rough screen width, grouped into one of four sizes rather than recorded exactly
- A random number that identifies the browsing session only. It is held in your browser’s sessionStorage, not in a cookie, and your browser discards it when you close the tab
That is sent to our own domain and recorded in our website hosting provider’s server logs. We use no advertising, social-media or analytics-company tracking, and we set no cookies of our own. The measurement does not record your name, your email address, your phone number, or anything you type into a form, and our beacon does not record your IP address or your browser’s user agent — although, as with any website, our hosting provider necessarily sees your IP address for as long as it takes to serve you the page.
We do not use the pages you read to identify you. A visit to a page about opioid or alcohol treatment tells us that the page was read. It does not tell us who read it, and it is never linked to a patient, a chart or an appointment.
These logs are kept only for the short period our hosting provider retains server logs. We do not copy them into any other system, we do not build profiles from them, and we do not sell or share them.
The contact form. The form on our contact page asks for your name, phone number and email address, how you prefer to be contacted, which treatment you are asking about, and — optionally — the best time to call, your current treatment status and a message. We use those details only to answer that inquiry. To reach us it passes through two service providers that handle the message while it is in transit: Resend delivers it to us as an email, and a copy is also addressed to an email-to-fax provider, Fax.Plus, for the clinic’s fax line. It is not published, sold, or used for marketing. The form is not a medical record and is not part of your chart. Personal health information for your care is collected in the clinic, under PHIPA, as described in section 1.
The contact form is not monitored around the clock and must not be used in an emergency. If you or someone else is in danger or may be overdosing, call 911. For free, confidential support 24/7, call ConnexOntario at 1-866-531-2600.
Map. Our home page and contact page embed a Google Maps view of 55 Dundas St E. When that map loads, your browser contacts Google directly. Google may receive your IP address and may set its own cookies, under Google’s privacy policy rather than ours. We receive nothing back from it.
6. Contact Us
If you have any questions, concerns, or requests regarding your personal health information or this Privacy Policy, please contact us directly. We are committed to responding to all privacy inquiries in a timely and respectful manner.
Privacy Contact Information
This Privacy Policy may be updated periodically to reflect changes in legislation or clinic practices. The effective date at the top of this page indicates when the policy was last revised. Continued use of this website is subject to the version of this policy in effect at the time. That is not, and does not replace, your consent to the collection, use or disclosure of your personal health information for your care — that consent is discussed with you in the clinic, under PHIPA, as described in section 1, and you may withdraw it as described in section 3.
